Medical Travel Assistance Program

Provides assistance to eligible Muscogee (Creek) citizens who must travel over 25 miles to receive medical treatment. Treatment includes, but is not limited to, cancer treatment, dialysis, major surgery, or mothers who deliver prematurely, etc. Assistance provided includes reimbursement or pre-payment with toll fees, parking fees, gas, food and/or lodging. Assistance may be paid up to $2,000 a lifetime. Cancer patients may be eligible up to $10,000 a lifetime and is considered on a case by case basis. Guidelines apply.

APPLICATION

TYPES OF MEDICAL TREATMENT OR FOLLOW UP APPOINTMENT:
Treatment includes, but is not limited to:

  • Cancer Treatment
  • Dialysis
  • Major surgeries
    • Brain
    • Heart
    • Removal of tumor
    • Removal of limb
    • Transplant
  • Hip Replacement
  • Knee Replacement
  • Premature Labor/Delivery
    • Women who have a premature delivery shall be eligible for assistance. Mother and father of the baby who drive back and forth from home to the hospital will be eligible for assistance.

TYPES OF MEDICAL TREATMENT NOT CONSIDERED:
Treatment includes, but is not limited to:

  • Cosmetic Surgery
    • Botox
    • Breast
    • Lip Injections
    • Facelift
    • Liposuction
    • Gastric Bypass
    • Dental
  • Routine doctor or dentist appointments.

BASIC REQUIRED DOCUMENTATION:

  • Copy of citizenship card or printout from the MCN Citizenship Office for all household members. Driver license/ID or birth certification will be accepted for non-Indians.
  • Copy of social security card for all household members.
  • Documentation of medical treatment/diagnosis.
  • Documentation from doctor – must list date and time of an appointment, and if overnight stay is required.
  • Applicant must keep all receipts from tolls, parking fees, gas, food and/or lodging, etc., to be reimbursed. If the applicant receives a pre-payment, all receipts must be turned in for the amount awarded. Should the applicant not spend the amount awarded, the remaining funds must be turned in with the receipts. If funds or receipts are not turned in within 10 days of the last day of the treatment/appointment, that amount will be deducted from the amount to be awarded on the next application. Should the applicant not comply with program guidelines, they shall not receive assistance for one (1) year. Abuse of the program shall be determined by the Social Services Director.

APPLICANTS MUST ALLOW 1-2 WEEKS FOR PROCESS

APPLICATION PROCESS:
Once all required documentation has been received, the social worker has three days to complete the application.
