School Clothing Program

 

COLLEGE/VO-TECH SCHOOL CLOTHING GUIDELINES

Eligibility:

  1. Student must be enrolled with Muscogee (Creek) Nation
  2. Student must be 24 years of age or younger
  3. Enrolled full-time in a college or technical school
  4. Pursuing an undergraduate education/certification
  5. Student must complete the application

Guidelines:

  1. MCN College/Technical School Clothing grant is $150.00
  2. Assistance is based on funding availability
  3. Apply once per year, January 2018 through December 2018
  4. Reimbursements are not allowed
  5. Purchases are not tax exempt
  6. The school clothing check is void after 60 days from the date of the check
  7. Once the check is cashed by the applicant, the Muscogee (Creek) Nation/School Clothing Program are absolved from any further responsibility.

Required Documents:

  1. Completed application – IF NOT COMPLETE, YOUR APPLICATION WILL BE MAILED BACK TO YOU.
  2. Copy of citizenship card (CDIB cards are not accepted.)
  3. As proof of enrollment, please submit a letter on school letterhead from the registrar showing full time enrollment and including the official seal of the college or technical school.

 

Receipt Guidelines:

  1. Due within 90 calendar days from the date on the check, not the date the check is cashed.
  2. Postmark dates will be accepted. Postmark dates must be legible and have the date printed.
  3. Receipts must be dated AFTER your check issuance date. This is not a reimbursement program.
  4. Receipts must be grouped and labeled by student OR combined receipts must be clearly marked showing amount spent per student.
  5. Receipts are monitored for non-compliance.
    Do not submit cash, coins, check or money orders or other forms of payment with receipt.
  6. Copies, scanned or original receipts will be accepted. (Do not submit photos of receipts)
  7. Legibly print the head of household name, student’s name and phone number on each receipt.
  8. All receipts must be dated, itemized, and have the store name and phone number. Do not cut off any information.
  9. Garage sale receipts will not be accepted.
  10. School receipts (clothing purchased from a school) must be itemized, dated, stamped with school stamp and signed by a school official.

*** Failure to comply with receipt due date and amount will result in student being ineligible for the following program year. Exceptions to this will include a natural disaster or medical/family emergency. Documentation of the natural disaster or medical/family emergency and a written appeal must be submitted within 10 calendar days from date of MCN’s notifications. Date of incidents must have occurred within 90 calendar days of the date on the check.

*** If receipts are not submitted or receipts do not total the amount distributed for the previous year, that child will not be eligible for assistance, regardless of who applied for the child.

Approved Clothing Items: Tops, pants, jeans/shorts, dresses/skirts, undergarments, belts, winter hats, winter gloves, scarves, coats, shoes, boots, coveralls. Any items purchased that are not listed on the approved list are subject to denial. Non approved items will not be counted toward your receipt total.

Non-Approved Items: check cashing fee, cell phones/minutes, make-up, perfumes, colognes, pajamas, sleepwear, swimsuits, sports gear/mouth pieces, shoe inserts, back packs, school supplies, hair accessories, jewelry, prom dresses, tuxedos, suits, formal wear, etc.

Due to number of applications, please allow 8 – 10 weeks for application to be processed.

 

Application can be emailed, faxed, mailed in or dropped off in the office.

Applicant must keep copies of receipts for their records.

All applicants must comply with all guidelines in order to be eligible for the next school year.

The School Clothing program starts January 3, 2018 and ends December 28, 2018.

The School Clothing Assistance program is open to all enrolled Muscogee Creek students, regardless of where you reside.

  1. Completed application – IF NOT COMPLETE YOUR APPLICATION WILL BE MAILED BACK TO YOU *Completed application includes pages 1 and 2, school enrollment form and citizenship card if applicable.
  2. If copy of citizenship card was NOT submitted in 2017, please attach a copy of student’s citizenship card. (CDIB cards are NOT accepted.) If the copy of card you previously submitted has expired, you must submit an updated copy of the citizenship card.
  3. School enrollment form that is attached to the application. -IF NOT COMPLETE, THE FORM WILL BE MAILED BACK TO YOU. Schedules, demographics, report cards, letters on school letterhead, etc. will not be accepted.
  4. Verification of guardianship or other documents may be requested as needed.

  10. Receipts will NOT be accepted if combines with other purchases (ex. toiletries, food, non-clothing items) highlighters are used on the receipts, receipts are stapled or taped, the date is not on the receipts. NO EXCEPTIONS.

Eligibility:

1. Student(s) must be enrolled with the Muscogee (Creek) Nation.
2. Parent/Guardian applying for the child, must be the legal/custodial parent, legal guardian, foster parent or caseworker at the time the application is processed in the school clothing office.
3. Students must be enrolled in Pre-K through the 12th grade OR 3 years of age and enrolled in a certified Head Start Program, at the time the application is received.
4. Home School students must be 5 -18 years of age. If a child tests out before turning 18, the child will no longer be eligible for the program.

Custody Order, Legal Guardianship, or Power of Attorney filed in a court

• Should documentation listed above not be available, the following will be accepted: Proof of residency letter on school letterhead from a school official verifying the said guardian on application is listed as the guardian (main contact) with verification of the child(rens) home address.
• If a Custody Order, Legal Guardianship or Power of Attorney should change, no additional payments will be issued to the current guardians. ($200 per student, per year.)
• If custody is joint (50/50) and a primary, custodial parent is not listed on the documentation; the parent who applies first will receive the assistance.
• Notarized or hand written statements will not be accepted.

APPLICATION PROCESS

Only one (1) application per household will be accepted.

• Application may be submitted by mail, fax, email, in person or by dropping it in the drop box located at the School Clothing Office. Please do not mail, fax and email duplicate applications.
• If a household is applying for both biological children and foster children, a separate application will be required for each.

Receipt of Application Postcard

• Once an application is received, a postcard will be mailed to the head of household.

Pending Letter

• Incomplete applications will receive a pending letter and will be held until the end of the year. If needed documentation is not received by end of year, the application will be denied.

Processing Time

• Due to the number of applications received, please allow 8-10 weeks for processing and payments to be issued.

backpacks/school supplies
hair accessories, etc.

Any items purchased that are not listed on the approved list are subject to a denial. This denial will result in the amount purchased being decreased. Please note, receipts must total or exceed the amount awarded to be eligible for the next school year.
